In Section 5 of the NIP at the bottom where you sign and date, there are boxes asking for email and phone number, but nothing to say if they're optional.Are these details mandatory?

I've received a NIP for 36mph in 30mph zone on the North Circular, London. Should the actual Local Traffic Order be specified on the NIP for it to be valid? It just vaguely says 'a Local Traffic Order':Offence: Exceed a 30MPH speed limit in contravention of a Local Traffic Order - automatic Camera deviceIt then gives date, time, location and recorded speed.Thanks.
