The below PCN was issued to my father when he attended Waltham Forest Feel Good Centre with my mother to process his gym membership following a referral from his GP.

At the time, they were parked in a disabled bay with a valid Blue Badge clearly displayed. However, he was unaware that he also needed to either pay for parking or validate the vehicle registration at reception. From reviewing the PCN timings, it appears he was on site for only around 20 minutes.

Having looked through previous advice relating to PCNs issued at WF Feel Good Centre, the general recommendation seems to be to submit an appeal as the registered keeper without identifying the driver. However, when attempting to complete the Gemini appeals form, there does not appear to be an option that clearly matches this. I have attached a screenshot of the appeal options available but I am unsure which option would be most appropriate to select.

I have also contacted WF Feel Good Centre directly to request assistance with getting the PCN cancelled, as there should be a record confirming attendance at the centre at the relevant time, but so far I have not yet received a response.

Please find attached copies of the PCN along with the screenshot referenced above. I would be grateful for your advice on how best to proceed.

Thanks
